DOG LICENSE APPLICATION

 

All dogs residing in the Town of Lancaster must be licensed as required by state law. 

You will receive a license certificate and a numbered tag to be placed on the dog collar,

 which helps with the recovery of your dog if lost.

 

INSTRUCTIONS:

  1. Print the application to complete.

  2. All fields must be completed or the application will be rejected.

  3. Please make sure all information is complete and legible.

  4. Enclose a check or money order made payable to the Town of Lancaster.

  5. Include a self addressed stamped envelope for the return of the certificate and tag.

A copy of the rabies certificate and neutering or spaying certificate, if applicable,  must be included

with the application or it will be rejected.

                           1st dog of Senior Citizen (65 or over): $2.00
